Participants interested in attending either the Youth Program or the Adult Program must fill out the general
Registration form (Link Below). The completed form and fees must be received by Middletown Parks and
Recreation prior to any individual participating in the program. All participants must complete a swim test,
either offered by the Parks and Recreation or completed on your own. IMPORTANT, if participants complete
the swim test on their own, they must have it supervised by either a Lifeguard, Water Safety Instructor, or
equivalent. Once completed, the participant must sign and have the supervising party sign the wavier and give
it to the head coach on the first day of the program. Below is a link to the complete Middletown Parks and
